Chronic Disease Management

Chronic diseases are becoming more common amongst us and affect the way we work and live.

GP Chronic Condition Management Plans (GPCCMP) are designed to improve the management of long-term conditions by providing a clear treatment pathway and facilitating multidisciplinary care.

What qualifies for a GPCCMP?
A chronic medical condition is defined as a condition that lasts for six months or more. It is a health issue that requires long-term management, often involving multiple health services, including specialists and allied health providers.

  • Mental Health Conditions

Diabetes Management

Our skilled team of GPs and Practice Nurses can help you or a family member manage your diabetes (Type 1 & 2).  Every person is different and every treatment needs to be personalised to ensure you continue living a healthy and long life.  Our services include:

  • Blood glucose monitoring – tracking your blood glucose levels as advised by Wheatfields Medical GP

  • Medication prescriptions – as prescribed by your GP, medications such as insulin can maintain target glucose levels.

  • Regular screening – Monitor blood pressure, cholesterol, kidney function, as well as have annual eye and foot examinations.

  • Weight management – Maintaining a heathy weight range can significantly reduce your risk of being diagnosed by diabetes and help improve the health of people with diabetes.

Health Assessments

A health assessment is a comprehensive, proactive evaluation of your physical psychological, and social well being. It goes beyond a standard consultation, utilising physical examinations, taking a medical history and lifestyle reviews to identify risks early, track chronic conditions and establish preventive care plans. The appointment is booked with one of our Practice nurses and the report is then given to the GP who alongside the nurse, work together as a team to improve the participants health and wellbeing.

75+ Health Assessments
An annual comprehensive health evaluation for older adults to identify ongoing care needs.

Mental Health

Our mental health services are designed to provide the compassionate, professional care to individuals facing a variety of emotional and psychological challenges. We offer a range of services to support mental well being, including working closely with our mental health partners to provide holistic approach, to ensure that each patients care plan is personalised to their needs. Veterans Affairs CVC Plans

The CVC program provides proactive care coordination for eligible Veteran Card holder with chronic conditions and complex care needs. Nurse, GP’s and participants work as a team to improve the participant’s health and wellbeing and reduce hospitalisations.
